22岁孝子捐肝救癌母 成就首宗移植治愈晚期癌
（新加坡17日讯）22岁孝子捐肝救癌母，在超过50人的国大医学组织医疗团队协助下，成就了新加坡乃至东南亚首例通过肝移植有望治愈晚期结直肠癌的突破性创举，母子手术7天后双双痊愈出院。
2020年10月份，59岁的林意鸣（如今61岁）因为突然腹泻伴随腹部剧痛，到黄廷方综合医院做检查，不料被确诊为第四期即末期的结直肠癌。当时肿瘤已经大面积转移到她的肝脏，这类情况统计学上的五年存活率少过20%。约4成的晚期结直肠癌患者出现肝转移现象，也就是指癌细胞已扩至肝脏部位。
林意鸣昨天受访时忆述：“那天我是一个人去拿检查结果，因为我没想到（病情）会这样严重。我听到这个噩耗时，马上流下了眼泪，因为我想到我的儿子年纪还小。”确诊癌症前，林意鸣为抚养她的独生子，一直四处打零工，曾做过清洁工等不同工作。

在国大医学组织例常举办的内部肿瘤会诊委员会上，国大医院肝胆胰外科顾问医生邦尼（Glenn Bonney）副教授和他的团队了解到林意鸣的病情，并提出她可以考虑换肝，有望“除癌”。
林意鸣病情的标准治疗方法是手术切除肿瘤，配合化疗。而对于不可切除的结直肠癌肝转移患者，可采用的治疗手法就是进行肝脏移植治疗法，近年在一些西方国家如果挪威、芬兰等，创下术后83%的2年存活率。国大医院器官移植中心内科主任兼顾问医生穆迪亚医生（Mark Muthiah）透露，根据文献这在整个东南亚尚未有先例。
吴俊乐捐出60%肝脏
也是国大医院器官移植中心成人肝脏移植计划和国大癌症中心肿瘤外科顾问医生的邦尼（Glenn Bonney）副教授和团队，经过历时两年的商讨，确定林意鸣符合上述肝移植治疗的标准。
林意鸣的儿子吴俊乐（22岁）随即配型成功，成为母亲的活体捐肝者。今年7月28日，母子俩在超过50名医学专家和医疗人员的帮助下，成功完成肝移植手术，吴俊乐为妈妈捐出自己60%到70%的肝脏。
二人术后7天痊愈回家
由于此前林意鸣直肠的肿瘤已被切除，换肝后体内的癌症一并消失。二人术后7天痊愈回家，创下国大医院器官移植中心术后最快出院的记录。
吴俊乐告诉记者，当自己得知可以通过捐赠肝脏救妈妈的命时，他感到欣喜和如释重负。“我一定会捐肝给她治病，因为她是我的妈妈。”
林意鸣则表示，自己万分感激一路陪伴的医护人员。“我没有惧怕这个实验性的手术，因为我完全相信我的医生们。多亏他们和我勇敢的儿子，我重获新生。”
林意鸣也对期间受到的院方和政府的经济帮助表示感激，很大程度上帮助她的家庭负担了化疗和手术的昂贵费用。
肝移植风险大 不考虑70岁以上病人
要通过这个突破性手术治愈，前提是第四期结肠直肠癌患者必须对化疗有良好反应，而且得符合44项标准。这44项标准，由邦尼副教授和团队联合国大医学组织和国际上数十位专家共同制定。
这44项标准包括转移到肝脏的肿瘤无法通过手术切除、肿瘤要通过一系列明确的基因测试等等。
另外，现阶段不考虑70岁以上病人，因为肝移植风险太大。
目前，有约10%的第四期结直肠癌患者符合该标准。邦尼副教授表示，随着该领域技术的完善和经验累积，这44项标准将被简化，让这个治疗方式惠及更多的病人。
